In response to your request for a public IP address, ImOn asks that you complete the short form below. ImOn will review your responses and notify you if and when your public IP address will be made available to you. Please consider the following before requesting your public IP address:

• Because public IP addresses are a finite resource, ImOn must pay to acquire them, and therefore pass that cost through to customers who request them. The fee for one residential public IP address is $5 per month.
  
  
• ImOn's residential customers are protected from DDoS (Distributed Denial of Service) attacks. If your request for a public IP address is approved, your broadband connection will be moved from this protected Internet service to the public IP address service you are requesting.

• I certify that the use of a public IP address continues to adhere to ImOn's Acceptable Use Policy.
• I acknowledge that by choosing a public IP address, my residential Internet connection will no longer receive ImOn's DDoS protection.
• I agree to an additional $5 per month charge for the public IP address.
